About us Resources and reports Independence and Wellbeing Through Community Access Program When Liam*, 25, sought support from the Community Access Program (CAP), he had recently been discharged from hospital due to an unmanaged medical condition and was living in a challenging environment with friends. Liam was seeking assistance with housing, health and wellbeing, career options, and obtaining his driver’s licence. Through CAP, Liam was connected to housing, health, and mental health services to address his immediate needs. He also received career counselling, training opportunities, and driving support to build independence and pursue his personal goals. With ongoing guidance, Liam now manages his medical condition effectively, leading to immediate improvements in physical health. He has gained confidence, improved daily functioning, and has an increased sense of hope for the future. Liam has also gained his learner’s licence and identified career pathways, helping him continue building independence and motivation. CAP, coordinated by the Hervey Bay Neighbourhood Centre, provides personalised support for young adults facing complex challenges, helping them achieve short-term goals while building skills, independence, and wellbeing, HBNC’s Client Support Manager, Narelle Duroux said. “Liam’s journey demonstrates the transformative impact of tailored support and access to services,” Narelle said. “By focusing on practical goals and personal growth, CAP helps clients gain the confidence and skills to thrive.” *Name changed for privacy.
