Mental Wellbeing Support and Service Navigation

Service navigators can support you to access community services, tools, training and resources.

We can help you with:

  • Assistance with forms and linking with services
  • Transport to linked services
  • Information and referrals
  • Advocacy for Services and Programs

Independence and wellbeing through CAP

When Liam*, 25, sought support from the Community Access Program (CAP), he had recently been discharged from hospital due to an unmanaged medical condition and was living in a challenging environment with friends.

What We Do

HBNC is getting out and about in the community with its Meet the Neighbours Outreach Trailer, and we're inviting you to come along and join the fun. The trailer will be fitted out with over 25 fun games and activities, including giant chess, snakes and ladders, pong, twister, and various sporting activities. There will be something for all ages, young and old.  Upcoming dates coming soon!
