About us News One Year of Connection and Wellbeing: HBNC’s Social Prescribing Programme Reaches Milestone HBNC is celebrating a successful year of its pilot Social Prescribing Program, Connecting Communities, which has impacted the lives of Fraser Coast residents by fostering social connections and improving overall wellbeing. Launched in partnership with Fraser Coast health and community organisations, the program has helped clients overcome social barriers, rediscover purpose, and find meaningful community connections. Over the past 12 months, HBNC’s Social Prescription Team has worked closely with clients to match them with activities, groups, and clubs that align with their unique interests and needs. Clients receive personalised support and, where needed, team members can accompany those who feel hesitant to attend an event or activity alone. Maureen Hall, HBNC's Manager of Aged and Inclusion Services, said social prescribing was a powerful way to improve health and wellbeing beyond traditional medical care. “By connecting people with social opportunities and community activities, we are helping reduce feelings of loneliness and isolation, which can have a huge impact on mental and physical health,” Maureen said. Karla Steen, HBNC’s Social Prescription Team Leader, said clients had benefited in various ways, whether they were grieving, adjusting to a new medical diagnosis, or seeking connections after moving to the area. “Our goal is to provide pathways that help people feel engaged and valued in the community. We’ve seen clients who felt unsure about where to start now feeling a renewed sense of purpose and belonging,” Karla said. The reasons for client referrals into the program vary, often addressing factors such as: Coping with the loss of a loved one Experiencing loneliness or isolation due to physical barriers or anxiety Seeking purpose after a job change or retirement Managing a new medical diagnosis or injury Recently relocating and wanting to make connections Desire to engage in physical activities for improved health Exploring local activities and opportunities Health professionals and community members can refer clients to the Social Prescribing Program through the HBNC website.
