Services Connecting Communities: Social Prescription Who we can help The Connecting Communities Social Prescription helps residents over the age of 18 to connect to social events, clubs, groups and activities in a bid to reduce social isolation and loneliness and improve health and wellbeing. We offer free one-on-one support to Fraser Coast residents who would like to: Find out what is happening in their local area Be connected to local clubs, groups or activities Find exercise classes to join Improve their health and wellbeing Try something new Reduce social isolation or loneliness, or Engage in community service or volunteering. Refer a patient or client Self-refer Get in touch REPORT: 12 months of Social Prescription Since its launch in 2024, Connecting Communities Social Prescription has made its mark on the Fraser Coast. This bold new way of addressing health challenges has helped us think differently about loneliness and isolation, and the benefits of creating connection in our community. We are excited to introduce this report, created in partnership with University of the Sunshine Coast. You're invited to discover how Social Prescription has impacted our community. READ THE REPORT Finding connection and purpose through volunteering When Sarah* moved to the Fraser Coast, she was looking to make new connections and learn more about her new community. READ MORE How we can help Social prescribing is proven to improve health outcomes by addressing the issues of social isolation and loneliness through engagement with social and physical activities. The program is for those who would like some support to grow social connections or try a new activity. Participants may have recently moved to the region, had a change in life, are feeling lonely, or socially isolated, want to try something new or not know where to find information. Our program team will work closely with participants over a couple of sessions to help create new connections, boost your confidence and overall health and wellbeing**. Participants of the Social Prescribing Program are also able to join our monthly Chat with Friends - Chatty Cafe to meet other locals, hear from interesting speakers and find out about the wide range of activities, groups and clubs in our region. More details on these monthly catch ups please visit our What's on Page. **Please note we are not trained counsellors or offer counselling however we can provide a list of local organisations who may be able to support you. To meet with one of our Link Workers for social connection contact us HERE How it works Once you join the Connecting Communities program you will be introduced to one of our friendly Link Workers, who will work one-on-one with you to identify your interests and what you might like to do socially. You will then be supported by our team members to access activities, groups or other services with the aim of providing long-term friendship, engagement and purpose. Our staff may also accompany you to any activities to support you to feel comfortable in the new environment.
